Course information

Session 1 & 2 will be delivered together on 19.1.23

Session 1

• To gain an understanding of what Personalised Care & Support Planning (PCSP) is

• To be able to identify the principles, values and key criteria of PCSP

• To recognise the importance of holistic assessment in relation to PCSP

• To consider what matters to you and to the residents’ in your care

Session 2

• To gain an understanding of skills and behaviours needed to find out what matters to a person and capture it on record

• To recognise person-centred communication at End of Life and the skills needed

• To be able to identify elements of personalised care that will support the tailoring of PCSP conversation

Session 3 & 4 will be delivered together on 2.2.23

Session 3

• To gain an understanding of skills and behaviours needed to find out what matters to a person and those close to them

• To recognize and identify the importance of Empathy as a clinical skill

• To be able to sensitively and compassionately explore residents / families worries and concerns

• To consider who should be involved in PCSP

Session 4

• To consider where and how we document and share with the wider team within our practice and outside of our practice “ACP” (PCSP conversations)

• To gain an understanding the personalised information on emergency health care plans - ReSPECT

• To be able to support Important discussions including conversations about death and dying and resuscitation discussions
