Latest news from the Community Specialist Palliative Care Team- December 2024

We are delighted to bring you the latest edition of the Integrated Community Specialist Palliative Care Team (ICSPCT) Newsletter. This publication is your go-to resource for updates, stories, and insights from our team, as we continue to provide exceptional care, compassion, and support to those in need across our community.

Clinic pilots

City CNS hub is currently piloting a specialist nurse led clinic at LOROS Hospice every two weeks, for patients and their relatives/carers (where appropriate). The aim of these clinics has been:

• Seeing patients in a timely manner after referral

• Seeing more people in one day

• Full assessment of needs in a safe environment

• Direct access to electronic systems to check history, review results and make onward referrals

Access to urgent anticipatory medications

A new model for the urgent supplies of medication is now in place and went live on 1st October 2024.

The supplier of urgent end-of-life medications is Omcare and the dedicated number for them is 07706 651 053.

The service ensures delivery of medications to the patient's home within three hours of the prescription being received and runs from Monday-Friday, 7am-11pm. The provider is open on Saturdays and Sundays and is happy to help with supplies at weekends if this is required although delivery may not be available.

Prescriptions should be sent via the new mechanism that has been set up on SystmOne and Emis. Guidelines and information about this service and the changes to authorisations can be found on the LLR Training hub website.

Charnwood Ladies Lunch

Kelly Richardson attended the Charnwood Ladies Lunch. Kelly said: "I was invited to attend the Charnwood ladies lunch in November to discuss my career to date and LOROS services. I was made to feel very welcome and delivered a talk on my career starting in 2005 to date. I talked about my story behind the reason I wanted to specialise in palliative care."

"The feedback that I received on the day was that it was gratefully received, my passion and enthusiasm shone through. Also they did not realise LOROS offered so much support to patients and their carers and it was not just somewhere people go to die."

PAVE Conference

Our LOROS volunteering team attended the PAVE event at DMU a few weeks ago. A big shoutout to Natasha who was a brilliant advocate for the clinical team at this event! It was great to have her at the event talking all things recruitment.

Non medical prescribing course

Congratulations to Kelly, Amy, Julia, Hollie, Mia, Sally, Tendai, Lisa and Priti for starting their non-prescribing medical course and achieving success in their first exam. What a fantastic start to this journey!
